Scaling Choosing Wisely Through Quality Improvement in Primary Care

How can primary care practices move Choosing Wisely Canada’s recommendations from awareness to sustained practice change? The Calgary Foothills Primary Care Network (CFPCN) took a proactive approach by embedding Choosing Wisely principles into quality improvement (QI) initiatives across multidisciplinary teams within their Network and the 103 Patient Medical Homes it supports.
Co-designed with patients and delivered through Practice Facilitators and multidisciplinary teams, the initiative targeted three high-impact areas: reducing routine blood work, using fracture risk assessment before bone density testing, and deprescribing PPIs. Ready-to-use toolkits combine evidence and step-by-step QI guidance to enable rapid real-world implementation. Spread across Patient Medical Homes and amplified through peer-to-peer learning with a QI World Café event, this approach is driving measurable reductions in low-value care and enabling sustainable practice change.
