Dr. Moser will discuss avoiding unnecessary transfers to hospital for long-term care (LTC) residents and describe efforts to provide increased medical care and supports in LTC. Dr. Brian Wong, General Internist at Sunnybrook Health Sciences Centre, will provide reflections and commentary following the presentation.
