Many older adults are often prescribed multiple medications for different health conditions. While some of these medications are necessary, polypharmacy and inappropriate medication use can be harmful and result in poor outcomes in older adults.

Dr. McDonald will review the results of the MedSafer study, one of the largest deprescribing studies that has taken place in the acute care setting in North America, and discuss how electronic tools and decision support may be harnessed to decrease the burden of polypharmacy in hospitalized patients. She will also discuss next steps for deprescribing in long term care and community dwelling older adults.
